Social Security Office

Not only have records of my visit to this office NOT been recorded, but also, I have been given an "Emergency Request to get Medicare Part B" back-Form. The Social Security Admin let me knowntoday, via the phone, that THAT FORM DOES NOT EXIST. I am beginning to think workers at THIS OFFICE, Hazeltine Ave Jamestown, NY, IS PURPOSEFULLY MALICIOUS to diabled and elderly recipients of this governmental assistance, I EARNED. I NEED PART B BACK!!!!!!!!! I WILL NOW WAIT ANOTHER YEAR BECAUSE OF THIS OFFFICES INCOMPETENCE.
